Meera completed her PharmD at the University of Maryland Baltimore in 2009 and went onto complete a clinical pharmacy residency with a focus in ambulatory care at the Boston VA Healthcare System. She has worked at the VA Maryland Healthcare System since 2011 as an ambulatory care clinical pharmacy specialist and appreciates the autonomy and the progressive nature of her practice in this setting. Her practice areas include Primary Care, as well as Rheumatology. With her interest in the VA’s Whole Health model of care, Meera also cofacilitates a mindfulness program for Veterans and is enrolled in a 2-year fellowship in Integrative Medicine through the University of Arizona Center for Integrative Medicine. She is passionate about addressing the issue of polypharmacy and has worked towards initiating and expanding a program at the VA Maryland Healthcare System to address this issue. She is excited to share with you some information regarding a new deprescribing collaborative and a novel tool for the identification and management of high risk polypharmacy patients within the VA setting.

2. List two potential interventions that may prevent patient harm.
3. Describe the role of pharmacist in optimizing medication safety.
